Enrollment Forms for the Incoming Class of 2022 & Transfer Students
The Office of the First-Year Experience will mail the Health Services packet of information and required forms to you in May. If you have questions after reviewing the packet, please contact Health Services at 518-580-5550 or firstname.lastname@example.org.
For your convenience we have included links to the health packets.
June 1 deadline for Summer Academic Institute students to submit all paper AND online student portal health forms.
August 1 deadline for all other students to submit all paper AND online student portal health forms.
Student Health Web Portal Forms
All incoming students are required to register and create a personal account on the Student Health Portal.
Click on link at right. To register enter Skidmore ID# (include the 00), birth date,
security question, and security answer. Remember the security answer exactly as entered
as this must match when you enter the security answer shortly to set your password.
Click "Register" and you will receive a message "Registration Successful" and to "Check
Your Email for Instructions on Obtaining a Password."
Go to your Skidmore email and check your Inbox or *Junk/Spam mail folder for an email from "StudentHealthPortal" with subject "Portal Registration." Click on the Link to Obtain Password. You will have to answer the security question exactly how you entered it earlier. Create password and verify password. Click "Set Password." You will receive a message "Password Confirmation Successful and click "Click Here to Login."
Keep in mind the server can "time you out" and does not save information if you remain inactive in the open form. Once you click SUBMIT at the bottom of the form, the form will be sent directly to Health Services and will disappear from your "pending forms" file. You will be contacted if further information is required.
If you encounter any difficulties in logging in to the portal, completing portal forms, or receive an error message contact Skidmore Health Services (not Skidmore IT Department) for any problems you incur. We will be glad to assist you. Please contact us at (518) 580-5550 or email@example.com.
If you experience problems with registering or completing the forms try using the latest version of the browser (Internet Explorer, Firefox, Google Chrome, or Safari).
*If you find the email in your Junk/Spam folder in Outlook, mark email as Not Junk:
1. Click on Junk Email folder
2. Right click on email you want to make Not Junk
3. Click Junk > Not Junk
4. A Mark as Not Junk dialog box will pop up, check the Always trust email from "firstname.lastname@example.org" box, click OK.
5. From now on, all emails sent from this sender will not be filtered to the Junk email folder.
Add email sender's address to the Safe Senders List in Outlook:
1. Click on Junk Email folder
2. Select email that you want to add the sender to the Safe Senders list.
3. Right click email and then select Junk > Never Block Sender
4. This email address will not be filtered to the Junk Email folder.
