Complete, signed, copy of parent(s) 2012 FEDERAL Income Tax return with copies of original W2's. If you are on extension to file provide us with a copy of Form 4868 which is required to be submitted to the IRS when requesting an extension to file. Attach copies of your W2's. |

onto self-service to accept, declined or reduce your Federal Direct Loans. The link to self-service
