Download
the PDF file below. Fill out the form, sign it, and return it to the
HPAC office (Dana 384). Doing so will:
- Give the
HPAC permission to set up a file in your name for your letters of
recommendation and other documents in support of your application.
- Allow you
to choose whether or not you would like to waive your right to examine
any letters of recommendation or documents the HPAC receives or writes.
