Pre-Interview Application Form

If you are planning to request a spring interview, you will need to submit the Pre-Interview Application Form and complete your HPAC file. You can download the form below. Fill out the form and return it to the HPAC office (Dana 384) by April 1st. A committee interview will not be scheduled until your HPAC file is complete. A completed file must include:

A completed interview form
at least 3 letters of recommendation from professors or health professionals
a copy of your up to date Skidmore transcript
a rough draft of your personal essay
