Pre-Interview Application Form

If you are planning to request a spring or early fall interview, you will need to schedule two advising interviews with the Health Professions Advisory Committee. These meetings will be scheduled after you submit the Pre-Interview Application Form. You can download the PDF file below. Fill out the form (attach Practice Vision Essay and Personal Statement) and return it to the HPAC office (Dana 172) by February 15th.

Upon receiving the form:

  • The HPAC administrative assistant will schedule the two advising sessions.
  • Two members of the Health Professions Advisory Committee will determine if you are prepared to apply to a health professional school.
  • Students who are prepared may request a full Committee interview.
