Parents' 2017 Federal Income Tax Return and W2's.

Submit a, signed, dated, legible copy of your parent(s)' 2017 federal tax return, including schedules A, C and SE(if applicable) and all W2's.  For identification purposes, write the student's name and ID number at the top of the first page.  If your parent does not have a copy, s/he may obtain one by contacting the IRS at 1-800-829-1040.
