Parent/Guardian Post-Acceptance Forms

Congratulations on your student's decision to study off-campus!  Below are links to important information and required post-acceptance forms to be completed by the parent/guardian. The required parent forms can be scanned and emailed to us as an attachment or mailed to our office. Your student will also receive this information as well as additional forms s/he must complete. We encourage you to speak with your student about the details of the program. We are more than happy to speak to you personally about your student’s upcoming off-campus study, if you find there are still some unanswered questions.
